Job DescriptionThe Campus School at Boston College is a private, publicly funded special education day school within the Lynch School of Education and Human Development on the Boston College campus. The Campus School educates students ages 3-22 with extensive support needs, including complex medical needs.
SUMMARY
The Life Skills Coordinator will carry out teaching, programmatic and consultation responsibilities. The Life Skills Coordinator will be responsible for the development and implementation of all aspects of Life Skills Programming, including teaching life skills classes, coordinating student objectives and programming with educational and therapy staff. The Life Skills Teacher/Coordinator will also develop appropriate transition plans for students ages fourteen to twenty one and will oversee the transition process
PRIMARY DUTIES AND RESPONSIBILITIES
Act as a support between/among Campus School, families, adult service agencies and providers in planning for and implementing transition to adult life for all students ages fourteen and older
Attend and participate in Individualized Education Program (IEP) meetings
Prepare goals, student assessments and quarterly reports, as well as complete the Transition Planning Form in the IEP
Maintain records and data for all Life Skills/Transition activities
Attend regularly scheduled classroom team meetings, general staff meetings and in-service trainings
Assist with referrals and the admissions process for students 14 years old and older
Assist in the transition process for students beginning at age fourteen through their transition at age twenty-two, including applying for guardianship, eligibility for Department of Developmental Services, accessing additional outside resources, and assisting families in finding appropriate adult day programs
Develop transition portfolios in conjunction with the student's service delivery team for students transitioning to adult day programs
Develop and implement Life Skills curriculum and programming, in conjunction with classroom teachers
Assume responsibility for planning and assisting students with job placements around the Boston College Campus
Assist with developing strategies for using adapted design, available technology and adapting devices as needed to increase student independence and participation in functional activities
Evaluate the effectiveness of components of the Life Skills/Transition program
Provide training to Campus School staff in skills/transition programming as needed
Assist in other student/program related duties as assigned

RequirementsFormal EducationMasters Degree in Special Education or related field preferred
MA Certification/Transition Certificate preferred
